- Community Outreach: Spend approximately 75% of your time visiting members in their homes or long-term care facilities, requiring flexibility, adaptability, and patience in various situations.
- Documentation: Accurately document contacts and case management activities in our prescribed system, maintaining confidentiality and adhering to UHG policies.
- Resource Development: Identify and develop resources to support our members' needs, ensuring timely and effective service delivery.
- Case Closure: Ensure all necessary steps are taken to close cases efficiently, maintaining high-quality service standards.
- Education: Meet one of the following requirements: Bachelor's degree in a human behavioral science field, 5+ years of relevant experience in LTSS, or a combination of education and experience.
- Technical Skills: Proficiency in MS Office, including Word, Excel, and Outlook, and access to reliable transportation for travel within assigned territories.
- Location: Reside in Custer, Fremont, or surrounding areas.
- Experience: Previous experience as a caseworker or case manager with an LTSS population in a private or public social services agency may be considered in lieu of required education.
