Compensation for this role is $50.34/hour. Actual compensation is determined by years of licensure.
POSITION SUMMARY:
The Inpatient RN Care Coordinator is tasked with identifying, evaluating, strategizing, and orchestrating, implementing, supervising, and assessing options and services throughout the care continuum for individuals within their designated case load.
Healthcare services are managed on behalf of the patient to facilitate the provision of care at the suitable level, thereby enhancing clinical outcomes and satisfaction with services, as defined by personalized care plans.
The Care Coordinator ensures that primary responsibilities among team members are clearly designated for each component of the plan, preventing redundancy and fragmentation.
The Care Coordinator evaluates and intervenes to address the clinical, intricate psychosocial/behavioral health challenges and requirements linked to hospital stays and proactive transition strategies.
Qualifications
REQUIRED QUALIFICATIONS:
Education:
An Associate's degree in nursing; a Bachelor's degree must be secured within 3 years of employment.
Experience:
A minimum of 1 year of nursing experience
Licensure/ Certification:
State Registered Nurse License
Other Skills/ Knowledge:
Exceptional interpersonal and communication abilities.
Proficient computer skills, including familiarity with Microsoft software.
Strong analytical and problem-solving capabilities, with a keen attention to detail.
PREFERRED QUALIFICATIONS:
Education:
A Bachelor's degree in nursing
Experience:
A minimum of 3 years of experience in patient care with progressively increasing responsibilities.
Experience in managing chronic, complex illnesses in adult and/or pediatric care settings.
Licensure/ Certification:
Care Management Certification
