The Care Management Associate role plays a vital part in supporting the Care Management team by facilitating effective transition planning and length of stay management.

This position requires coordination of information and communication, as well as other clerical functions for the Care Management team.

The focus is on performing duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.

As an independent and motivated individual with interest in coordinating and collaborating with healthcare team members, you will ensure patients receive the right information at the right time.

Job Responsibilities:

  • Collaborate with the interdisciplinary team to develop and implement effective care plans
  • Coordinate communication between patients, families, and healthcare providers
  • Maintain accurate and confidential records
  • Provide excellent customer service and support to patients and families
  • Stay up-to-date with industry developments and best practices

Requirements:

  • Bachelor's degree in Healthcare Administration or related field
  • 1+ year of experience in healthcare, preferably in a similar role
  • BLS (CPR) certification
  • Excellent interpersonal and organizational skills
  • Able to work independently and prioritize tasks effectively
  • Strong verbal and written communication skills
