Job Description

As a Clinical Social Worker, you will play a vital role in coordinating patient care services and resources to achieve optimal clinical quality and financial outcomes. Your key responsibilities will include:

  1. Collaborating with the Case Management Coordinator and other healthcare team members to provide seamless patient care.
  2. Utilizing screening mechanisms to identify patients at risk for lengthy inpatient stays, complex clinical issues, or high resource use.
  3. Intervening within 24 hours of patient admissions to ensure timely and effective care.
  4. Documenting according to established guidelines to maintain accurate patient records.

In this role, you will have the opportunity to make a meaningful difference in the lives of our patients and contribute to the delivery of high-quality care services.
