Job SummaryWe're seeking an experienced and compassionate Hospice Care Specialist to join our team in Kerrville, TX. In this role, you'll provide essential personal care services to patients, including bathing, grooming, and light housekeeping. You'll work closely with our interdisciplinary team to ensure seamless care coordination and timely communication with patients and families.
Responsibilities- Provide personal care services as assigned, including bathing, back rubs, oral hygiene, shampoos, and changing bed linen
- Assist patients with dressing and undressing as needed
- Prepare nutritious meals and assist with feeding as assigned
- Take and record vital signs, including temperature, pulse, respiration, and blood pressure
- Provide skin care and observation to prevent breakdown over bony prominences
- Report patient condition and significant changes to the Case Manager
- Collaborate with caregivers and other individuals living with patients
- Support ambulation and exercise as instructed by the hospice nurse or therapist
- Meet state training requirements for hospice aides
- Completed 75 hours of classroom and supervised practical training
- Minimum six months experience in providing personal care
- Ability to read and follow written instructions and document care given
- Satisfactory references from previous employers
- Ability to work with minimal direct supervision
- CPR certification
