Your primary focus will be on delivering high-quality, patient-centered care while collaborating with our team of healthcare professionals.
- About the Role:
- Provide palliative care consultations to patients and their families in their homes or other settings as needed.
- Collaborate with our multidisciplinary team to develop personalized care plans that address the unique needs of each patient.
- Work closely with patients and their loved ones to establish goals of care and ensure treatment aligns with their wishes.
- Demonstrate expertise in pain and symptom management, and provide education to patients and caregivers on self-management techniques.
- Education: MD or DO degree and board certification in Palliative Care or a related field.
- Experience: Minimum 3-5 years of clinical experience in palliative care or a related field, preferably in a home-based care setting.
- Licensure: Active, unrestricted medical license to practice in New Jersey.
