Job Summary

We are seeking a skilled and dedicated LPN to join our team at The Village at Willow Crossings. As an LPN, you will play a vital role in delivering high-quality nursing care to our residents, working closely with our interdisciplinary team to ensure their physical, emotional, and social needs are met.

Responsibilities

  • Assess, communicate, and document residents' status accurately
  • Observe and report any significant changes in resident behavior and health to the Resident Care Director, physician, family, RCAs, and Executive Director per state regulations
  • Direct and supervise the daily work assignments of the Resident Care Assistants
  • Closely mentor, guide, and direct the Lead Resident Care Assistants in all aspects of their job
  • Coordinate care needs with community providers via an effective case management process
  • Effectively balance service demands with supporting resident independence
  • Recognize and assist in the prevention of elder abuse, neglect, and exploitation (financial and other) and report to appropriate sources per state regulations
  • Respond to emergencies and personal emergency response system and know when to call for backup

Requirements

  • Current state license as an RN/LPN and CPR certification
  • Graduate of an approved RN/LPN program (per state requirements)
  • Minimum of 1 to 2 years of experience working as a nurse preferred
  • Previous experience working with the elderly in a paid or volunteer position is highly desirable
