Physical Therapist, Home Health Care Professional

We are seeking an experienced Physical Therapist to join our Home Health team at Sutter Health. As a Physical Therapist, you will work with patients in their homes, developing and implementing individualized rehabilitation plans that improve mobility, relieve pain, and enhance overall well-being.
This is a full-time position with a competitive salary range of $71.33 - $89.16 per hour. The successful candidate will have a strong foundation in physical therapy principles and practices, excellent communication skills, and the ability to work independently in a fast-paced environment.
About the PositionThis role involves working as part of a dynamic team to deliver comprehensive physical therapy services to patients in their homes. You will collaborate closely with other healthcare professionals to ensure seamless care transitions, communicate effectively with patients and families, and participate in ongoing education and professional development opportunities to stay up-to-date with best practices in physical therapy.
The ideal candidate will have a Master's degree in Physical Therapy from an accredited institution, current licensure as a Physical Therapist in California, and at least 5 years of experience in physical therapy, preferably in a home healthcare setting.
Responsibilities and Requirements- Conduct thorough assessments to identify patient needs and develop tailored rehabilitation plans
- Implement evidence-based interventions to promote optimal outcomes and improve patient function
- Maintain accurate and timely documentation of patient progress and treatment outcomes
- Collaborate with interdisciplinary teams to coordinate patient care and achieve shared goals
- Participate in ongoing education and professional development to enhance clinical skills and knowledge
- Must have a strong foundation in physical therapy principles and practices
- Excellent communication and interpersonal skills
- Able to work independently and as part of a team
- Familiarity with electronic health records and other relevant software systems
