Job Summary:

We are seeking a compassionate and dedicated Care Manager (Caregiver) to join our team at Sunrise Senior Living. As a Care Manager, you will be responsible for providing high-quality care and services to a consistent group of residents and their families in our assisted living and reminiscence neighborhoods.

Responsibilities:

  • Build Meaningful Relationships: Establish and maintain strong relationships with residents and their families, providing assistance with activities of daily living and attending to individual care needs.
  • Notify Management: Inform management of any changes in condition and recommend adjustments in the level of care and service as needed.
  • Life Enriching Activities: Assist residents in life skills and other life-enriching activities, as indicated on their individual profile.
  • Blending Experiences: Create a variety of multi-sensory experiences for residents, blending music, art, and other activities to enhance their daily lives.
  • Individualized Service Plans: Participate in the development and monthly updates of Individualized Service Plans (ISPs) to ensure residents receive tailored care and services.
  • Resident Care: Be responsible for a designated group of residents during shifts, knowing their whereabouts and physically checking on them throughout the shift.
  • Community Teamwork: Collaborate with the community team to ensure compliance with OSHA requirements and promote Risk Management programs and policies.
  • Dining Services: Respond to the dining needs of residents and guests, maintaining Sunrise hospitality and service standards.
  • Inviting, Encouraging, and Assisting: Invite, encourage, and assist residents in life skills and other life-enriching activities, as indicated on their ISP and demographic profile.

Qualifications:

  • Education and Certifications: High School diploma or GED accepted; may be required per state regulations; in states where applicable, must maintain certifications.
  • Experience and Skills: Previous experience working with seniors preferred; dedication to and passion for serving seniors with excellent customer service skills; positive attitude, flexibility, and ability to work in a team environment.
  • Age and Physical Requirements: Must be at least 18 years of age; able to perform various duties in service to residents.
