Clinical Care Specialist

ScionHealth is dedicated to delivering exceptional patient care and service excellence. As a Registered Nurse within our Acute Rehabilitation Unit (ARU), you will play a vital role in the recovery process for chronic, critically ill, and medically complex patients.
Key Responsibilities:
* Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary teams.
* Provide high-quality, patient-centered care through the nursing process of assessment, planning, intervention, and evaluation.
* Communicate effectively with physicians, nursing supervisors, and colleagues to ensure seamless care transitions.
* Participate in discharge planning and ensure a smooth transition of care for patients.
Qualifications:
* Bachelor's degree in Nursing or related field.
* Current state licensure as a Registered Nurse.
* CPR/BCLS certification.
* ACLS preferred.
* CRRN preferred.
