We are seeking a skilled Registered Nurse to join our team at Kaiser Permanente Parsons West. As a Hospice Nurse, you will provide high-quality care to patients in a home health setting.
Key Responsibilities:
Evaluate and address patient needs with sensitivity and care.
Coordinate with a multidisciplinary team to ensure quality patient care.
Maintain accurate and timely documentation of patient records.
Participate in quality improvement initiatives.
Communicate effectively with patients, families, and healthcare providers.
Manage average travel of 30 miles per day/week within assigned coverage area.
Ensure compliance with all state and federal regulations related to hospice care.
Requirements:
RN license
BLS certification
Hospice experience, quality experience, and OASIS experience preferred
