Job Title: MDS Coordinator

We are seeking a skilled MDS Coordinator to join our team at The Laurels of Bedford.

About the Role:

The MDS Coordinator will be responsible for overseeing the Resident Assessment Instrument (RAI) process, conducting assessments, and coordinating care plans for our guests. This role requires strong communication and collaboration skills, as well as the ability to work effectively with interdisciplinary teams.

Key Responsibilities:

  • Complete MDS, CAA's, and care plans within regulated time frames.
  • Coordinate scheduling of the RAI process with the interdisciplinary team.
  • Assess residents through physical assessment, interview, and chart review.
  • Discuss resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff.
  • Review information from hospital, consults, and outside agencies, and use such information in the completion of the assessment and care planning.
  • Coordinate, identify, and/or initiate significant change MDS.'
  • Be prepared to conduct PPS meetings, maintaining MDS assessments per Medicare schedule, and maintaining PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
  • Remain current with American Association of Nursing Assessment Coordinators (AANAC) requirements.

Qualifications:

  • RN or LPN
  • AANC certification a plus. RAC-CT
  • Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS) strongly preferred.
  • Experience as an MDS Nurse
