Pharmacist - Clinical Specialist

This position is located within the US Department of Health and Human Services, Indian Health Service, Pharmacy Department. The Pharmacist will provide clinical pharmacy services to patients, including medication therapy management, patient education, and disease management.
Key Responsibilities- Provide medication therapy management services to patients, including medication reconciliation, dosing, and monitoring.
- Develop and implement patient education programs to promote healthy behaviors and disease management.
- Collaborate with healthcare providers to develop and implement disease management plans.
- Conduct medication use evaluations to identify opportunities for improvement.
- Develop and implement quality improvement initiatives to improve patient outcomes.
- Pharmacist licensure in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
- Graduate degree in Pharmacy from an accredited institution.
- At least 1 year of experience as a pharmacist in a clinical setting.
This position requires frequent travel to remote areas and may require working in a fast-paced environment with multiple priorities.
