We are seeking a compassionate and detail-oriented Registered Nurse to join our Hospice of Alabama team as a Full-time Registered Nurse Case Manager. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional end-of-life care to our patients and their families.
Responsibilities- Assess patient needs and develop personalized care plans in collaboration with our Clinical Director.
- Provide emotional support and education to patients and their families, empowering them to make informed decisions about their care.
- Coordinate and manage patient care, ensuring seamless transitions and timely interventions.
- Collaborate with our interdisciplinary team to ensure comprehensive care and address any concerns or issues that may arise.
- Participate in call duties, providing 24/7 support to our patients and their families.
- Current unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice experience a plus.
- Computer proficiency in typing and various programs, including background in EMR.
- Possess and maintains current CPR certification if required by state.
