Primary Care Physician

Abode Care Partners is seeking a highly skilled Primary Care Physician to lead our clinical team in providing high-quality medical care to seniors and individuals with complex conditions.
Responsibilities:- Provide complex and geriatric care to residents and patients across various settings in partnership with non-physician practitioners and other interprofessional team members.
- Collaborate with facility leadership teams, nurse practitioners/physician assistants, patients, and families to ensure high-quality care.
- Provide collaborating physician duties for nurse practitioners and physician assistants to support them in their clinical roles.
- Audit medical orders and documentation for each provider monthly to ensure compliance with state and regional regulatory frameworks.
- Assume medical directorships for skilled and/or other senior living facilities as needed.
- Identify new growth opportunities with skilled facilities and/or other senior living facilities in the market/region.
- Participate in company frontline/backup call processes for timely response to acute changes of condition to prevent avoidable hospital transfers.
- Focus on quality outcomes, including care path compliance, productivity, prevention of avoidable transfers, and decrease in polypharmacy and costs of care.
- Graduate from an accredited school of medicine and a Doctor of Medicine.
- Two or more years of administrative leadership experience in healthcare.
- Clinical medical experience of seven years or more preferred.
- Board certification in Family Medicine, Internal Medicine, or Geriatrics; qualification in Geriatrics preferred.
- Practicing physician currently licensed under the provisions of the State Board of Medicine.
- Active unrestricted licensure to practice in the state of employment.
- Current Drug Enforcement Administration registration.
