Job Title: Home Health Registered Nurse - Full-Time Contract

Job Summary:

  • Provide compassionate care in both home and facility settings
  • Perform comprehensive patient assessments and develop individualized care plans
  • Administer medications and treatments as prescribed
  • Educate patients and families on end-of-life care, symptom management, and medication usage
  • Coordinate care with interdisciplinary team members and ensure continuity of care
  • Document patient care accurately and efficiently in Oasis & Homecare Homebase
  • Participate in on-call rotation, typically one weekend per month
  • Maintain compliance with all healthcare regulations and company policies

Requirements:

  • Minimum of 1 year of home health Registered Nurse (RN) experience
  • At least 2 years of overall nursing experience
  • Proficiency in Oasis and Homecare Homebase software
  • Valid Registered Nurse (RN) license in the state of West Virginia
  • Active West Virginia Cares
  • Excellent communication and interpersonal skills
  • Reliable transportation
