Key Responsibilities

  • Connect patients with community programs and resources to manage the physical, mental, emotional, and financial demands of End Stage Renal Disease
  • Collaborate with healthcare teams to develop personalized treatment plans
  • Conduct assessments and develop care plans to address patients' social and emotional needs
  • Provide education and support to patients and their families on disease management and treatment options
Requirements
  • Master's degree in Social Work (MSW) required
  • Licensed or registered in the state of Michigan as required by state regulations
  • Demonstrated knowledge of government and private insurance programs
  • Basic computer skills in MS Word, Excel, PowerPoint, and Outlook, with functional proficiency in DaVita specific applications within 60 days
  • Dedication to caring for patients suffering from chronic kidney failure
