Key Responsibilities:
  • Work collaboratively with a clinical care team to deliver high-quality care in the patient's home through our innovative in-home care service lines.
  • Perform comprehensive health assessments, diagnose, and treat complex illnesses, and provide therapeutic interventions as needed.
  • Use critical thinking skills and evidence-based standards of practice to ensure accurate and thorough documentation of patient encounters.
  • Communicate effectively with patients, families, and healthcare teams to ensure continuity of care and address any concerns or questions.
  • Identify and proactively solve problems to ensure seamless delivery of care.
Requirements:
  • Minimum of two years of experience as an APP in the Emergency Department, Urgent Care, Internal Medicine, or other relevant areas.
  • Graduate with an advanced degree from an accredited institution.
  • Current unrestricted state license as a Nurse Practitioner or Physician Assistant.
  • Nationally board certified, current.
  • Current BLS required, ACLS certification preferred.
  • Prescriptive authority and DEA.
  • Valid driver's license with a clean driving record.
  • Ability to lift and carry equipment up to 50 pounds.
  • Ability to walk up and down several flights of stairs easily.
