Key Responsibilities:
- Implementing physicians' prescribed plans of care for patients
- Providing skilled nursing care to patients in their homes
- Collaborating with interdisciplinary teams to coordinate patient care
- Documenting all patient interactions and care provided in a timely and accurate manner
- Participating in ongoing education and training to stay current with best practices and industry developments
Requirements:
- Degree in Nursing (RN, LPN, LVN) from an accredited program
- Active and unencumbered licensure in the state where employed
- Current CPR certification from American Red Cross, American Heart Association, or BLS
- 6 months of prior nursing experience preferred
- Reliable transportation and ability to travel to patient homes
