Job Description:

We are seeking a Licensed Practical Nurse (LPN) to join our team in Avondale. As an LPN, you will provide skilled nursing care/services to pediatric and adult patients in their own homes, utilizing the nursing process to assess, plan, implement, and evaluate patient care.

Responsibilities:

  • Utilize the nursing process to assess, plan, implement, and evaluate patient care
  • Provide skilled nursing care/services in accordance with prescribed orders on each assignment
  • Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition
  • Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses

Qualifications:

  • Currently licensed as an LPN/LVN in the state in which the LPN will practice
  • Current TB or Chest X-Ray
  • Current BLS card
  • One year prior professional nursing experience preferred

Estimated Salary: $60,000 - $80,000 per year based on experience and location.
