Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Paramount Senior Living. As an LPN, you will play a vital role in providing high-quality care to our residents, working closely with our interdisciplinary team to ensure their physical, emotional, and social needs are met.

Key Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Conduct thorough assessments of residents' physical and emotional needs, developing and implementing individualized care plans
  • Collaborate with the healthcare team to ensure seamless care transitions and effective communication
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices
  • Provide education and support to residents and their families on health-related topics and disease management
Requirements
  • Valid LPN license in Pennsylvania
  • Minimum 1 year of experience in geriatric care
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Strong attention to detail and commitment to quality care
