Position Overview

SUMMARY: The Clinical Dietitian is tasked with providing nutritional guidance to patients identified with dietary risk factors, particularly focusing on individuals managing obesity and diabetes. This role also encompasses responsibilities as a Diabetes Care and Education Specialist, overseeing a diverse patient panel within a collaborative healthcare team.

KEY RESPONSIBILITIES

1. Individual Nutritional Counseling

  • Conducts comprehensive assessments and follow-up consultations utilizing the Nutrition documentation within the Electronic Medical Record (EMR).
  • Develops personalized nutrition plans that establish realistic and achievable goals for patients, supplemented with educational resources.
  • Ensures patients receive a written copy of their goals to facilitate progress tracking.
  • Exhibits in-depth knowledge in specialized dietary areas.
  • Employs sound clinical judgment when recommending treatment options in collaboration with primary healthcare providers.
  • Evaluates the psychological needs of patients and refers them to appropriate counseling services as necessary.
  • Identifies social support needs and directs patients to relevant social services.
  • Addresses spiritual needs, offering spiritual care and support as appropriate.
  • Practices cost-effective healthcare by considering necessary tests and treatment recommendations.

2. Group Educational Sessions

  • Facilitates group educational workshops on targeted topics for patients and community organizations, utilizing existing or newly developed lesson plans and take-home materials.
  • Communicates effectively with participants to enhance their understanding of dietary practices.
  • Clarifies the role of a dietitian to group attendees.

3. Diabetes Education and Support

  • Works in conjunction with primary care providers to deliver additional education and support during dedicated diabetes management sessions.
  • Instructs patients on home glucose monitoring techniques using finger-stick and continuous glucose monitoring tools.
  • Provides training on insulin and GLP-1 injection techniques.
  • Coordinates monthly group medical appointments.
  • Monitors patients adjusting insulin dosages and offers guidance based on established protocols.
  • Engages in population management and data analysis.
  • Reports on productivity and key diabetes metrics to supervisors and designated personnel.
  • Collaborates with the Medical department to integrate Community Health Workers into diabetes care initiatives.

4. Community Health Initiatives

  • Advocates for nutrition and healthy lifestyle choices at community events, health fairs, and educational classes, preparing relevant materials for distribution.

5. Maternal and Infant Nutrition Education

  • Partners with obstetric teams to deliver essential breastfeeding education and nutritional counseling for expectant and new mothers.

6. Professional Communication and Development

  • Maintains open communication with colleagues regarding patient care, programs, and procedures as needed.
  • Seeks guidance and feedback from supervising physicians when necessary.
  • Responds promptly to urgent patient needs.
  • Engages appropriately with external healthcare professionals.
  • Proposes enhancements and contributes to the implementation of practice improvements.
  • Continues professional development through both informal and formal educational opportunities.
  • Performs additional duties as assigned by supervisors.

QUALIFICATIONS

  • Bachelor's degree in Food and Nutrition or Dietetics from an accredited institution, with completion of the required practice experience or internship as mandated by the Academy of Nutrition and Dietetics (AND).
  • Registered with the Commission on Dietetic Registration (CDR) at the time of employment or within six months, and obtain state licensure as a Licensed Dietitian-Nutritionist (LDN) thereafter.
  • Bilingual proficiency in Spanish and English.
  • Ability to maintain composure in high-pressure situations.
  • Strong verbal communication skills; proactive in clear and direct communication with team members.
  • Intermediate proficiency in computer applications and Microsoft Office Suite.
  • Demonstrated capability to manage multiple priorities and details effectively.
  • Proven track record of handling confidential information with integrity and trustworthiness.
  • Ability to integrate personal faith with professional practice.

WORK ENVIRONMENT

  • Requires proficiency in using computers, reading fine print, and navigating electronic records.
  • Must be able to lift or move items weighing up to 40 pounds and perform occasional stretching, reaching, stooping, or kneeling.
