Job Overview

We are seeking a highly skilled Psychiatric Rehabilitation Specialist to join our team at Changing Lives at Home Mental Health Inc.

This is an exciting opportunity to work in a dynamic and supportive environment, providing essential services to women who have recently been released from incarceration.

About the Role

The successful candidate will be responsible for facilitating psychiatric rehabilitation, behavioral support services, and crisis intervention for assigned clients. This will involve teaching and instruction, life skills training, encouragement of positive change, skill demonstrations and practice, promoting individual wellness, self-management, and recovery.

  • Providing PRP services to assigned clients as scheduled by the Company (supervisor) with accurate documentation in the company's electronic health record on the same day of the encounter by the close of business.
  • Attending staff meetings once per week or as scheduled.
  • Collaborating with the client's therapist, psychiatric nurse practitioner, and other persons involved in the client's care to ensure compliance with treatment schedules.
Requirements

To be considered for this role, applicants must possess a minimum of three years of experience providing direct support to adults with severe persistent mental illness and substance abuse disorders. A high school diploma/GED and AA degree or greater are preferred.

Additional requirements include:

  • Excellent organizational, prioritization, and interpersonal communication skills.
  • Ability to handle multiple tasks and meet critical deadlines.
  • Clinical background check and no criminal convictions or history that indicate behavior which pose a potential threat to clients and staff.
  • Current first aid and CPR certification.
  • Ability to follow CLH Clinic practice guidelines and policies.
