LPN Medication Care Manager

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Medication Care Manager at Sunrise Senior Living. As a Medication Care Manager, you will play a vital role in ensuring the highest quality of care for our residents by administering medications and treatments in a safe and organized manner.
Responsibilities- Administer medications and treatments to residents in accordance with their individualized care plans and physician orders.
- Document and maintain accurate records of medication administration, including dosages, times, and any adverse reactions.
- Conduct regular medication reviews and updates to ensure residents are receiving the most effective and safe treatment.
- Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each resident.
- Provide education and support to residents and their families on medication management and other health-related topics.
- Participate in regular quality improvement initiatives to ensure the highest standards of care are maintained.
- Current LPN licensure in the state of employment.
- Minimum of 1 year of experience in a long-term care or senior living setting.
- Strong knowledge of medication administration and management principles.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
