Key Responsibilities- Assess, diagnose, and manage the healthcare needs of chronically ill geriatric patients in our LIFE program.
- Collaborate with Primary Care Physicians and Medical Directors to develop and implement individualized care plans.
- Provide education and support to patients, families, and caregivers on disease management, medication adherence, and healthy lifestyle choices.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care.
- Communicate effectively with interdisciplinary team members, including physicians, nurses, social workers, and other healthcare professionals.
- Current licensure as a Certified Nurse Practitioner in the State of Pennsylvania.
- One year of experience working with the frail and elderly population or willingness to participate in extensive training as needed.
- Valid State driver's license with a clean driving record.
- Successful completion of background check, physical, drug screening, and TB screening.
- Experience working in a geriatric care setting or with a similar population.
- Knowledge of geriatric care principles, including fall prevention, medication management, and end-of-life care.
- Ability to work independently and as part of a team to achieve patient-centered outcomes.
