Nurse Practitioner

Pinnacle Home Care Inc. is seeking a skilled Registered Nurse to join our team. As a Registered Nurse, you will be responsible for planning, organizing, and directing home care services to meet the needs of individuals and families within their homes and communities.
Key Responsibilities- Conduct initial assessments of patients and families to determine home care needs.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Provide direct patient care, including administering medications and treatments as prescribed by physicians.
- Counsel patients and families on meeting nursing and related needs.
- Communicate effectively with healthcare teams, including physicians, therapists, and other healthcare professionals.
- Participate in on-call duties as defined by the on-call policy.
- Graduate of an accredited school of nursing.
- Current licensure in state and CPR certification.
- One to two years of recent acute care experience in an institutional setting.
- Home health care experience preferred.
