Job Summary

We are seeking a highly skilled and compassionate Hospice Patient Care Manager to join our team at Three Oaks Hospice. As a key member of our interdisciplinary team, you will be responsible for coordinating and supervising staff to provide high-quality care to our patients.

Key Responsibilities

  • Assist with patient referrals and admissions, ensuring a smooth and efficient process.
  • Manage direct patient care by assigning staff, scheduling, and resolving issues as needed.
  • Audit patient records and respond to compliance issues, maintaining regulatory and accrediting agency requirements.
  • Lead and develop clinical staff through effective hiring, performance evaluation, coaching, and delegation.
  • Collaborate with the Executive Director and Director of Clinical Services as needed.
  • Educate and counsel staff on agency policies and procedures.

Requirements

  • College degree; a Registered Nurse/BSN is strongly preferred.
  • 2+ years of hospice experience and supervisory experience required.
  • Knowledge of hospice regulations and proficiency in Microsoft Office suite.
  • Excellent communication skills and ability to travel independently.
