Job Summary

The Home Health Registered Nurse provides high-quality patient care in a Point of Care setting, working collaboratively with the healthcare team to achieve optimal patient outcomes. This role requires strong clinical expertise, excellent communication skills, and a commitment to delivering exceptional patient care.

Key Responsibilities
  • Clinical Competence
    • Develop and implement comprehensive care plans in collaboration with the physician and other healthcare professionals.
    • Supervise and direct the care provided by home health aides and licensed practical vocational nurses, ensuring adherence to state and federal regulations.
  • Documentation and Care Delivery
    • Provide high-quality clinical services within the scope of practice, adhering to infection control standards and payer requirements.
    • Accurately and timely complete comprehensive assessments, including medication reconciliation, and document patient visits per policy.
  • Quality and Professionalism
    • Ensure patients meet home health eligibility and medical necessity guidelines, accurately determining primary focus of care and developing plans within state-specific guidelines.
    • Communicate effectively with agency staff, physicians, patients, and family members to ensure quality care and service excellence.
  • Teamwork and Productivity
    • Participate in on-call rotation and weekend rotation as needed to meet patient needs.
    • Adhere to and participate in the agency's utilization management model.
Requirements
  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
