Key Responsibilities
  • Facilitate patient referrals to home health services, ensuring that patient preferences are respected.
  • Assess patient eligibility for home health services upon physician or facility request.
  • Receive and process referrals from assigned physicians or facilities, directing them to the appropriate branch of the home health agency.
  • Act as a communication bridge between patients/caregivers, physicians, and the home health agency.
  • Coordinate follow-up appointments with physicians as necessary.
  • Receive and relay patient reports from agency staff to the physician/nurse.
  • Communicate any physician orders or changes to the home health agency.
  • Ensure that home health physician orders are signed and forwarded to the agency.
  • Follow up to confirm that original orders are sent to the home health agency.
  • Assist in procuring Durable Medical Equipment (DME) after obtaining necessary prescriptions from physicians.
  • Organize the delivery of equipment and any required outpatient services.
  • Ensure proper physician documentation is obtained to meet home health encounter requirements.
  • Address any complaints or concerns from physicians with the Director of Nursing or Branch Manager.
  • Provide educational sessions on home health agency services to healthcare professionals.
  • Meet personal performance goals and admission targets for the location.
  • Participate in sales calls and company training sessions.
  • Perform additional duties as assigned.

Qualifications
  • Experience: Previous experience in Home Health is preferred.
  • Licensure: Must possess formal clinical training from an accredited institution and hold a valid license in the state.
  • Driver's License: A current driver's license, vehicle insurance, and access to reliable transportation are required.

Skills:
  • Exceptional presentation, negotiation, and relationship-building abilities.
  • Proficient computer skills, including Microsoft Outlook and CRM software.
  • Ability to work independently with minimal supervision.
