Job Summary

At Chambers Quality Health Care, we are seeking a skilled Licensed Practical Nurse to join our team. As a Licensed Practical Nurse, you will play a vital role in providing primary care to our patients.

Key Responsibilities:

  • Maintain confidentiality of patient information and adhere to HIPAA guidelines.
  • Perform baseline assessments of patients and prepare them for examination and treatment.
  • Administer medications and treatments as prescribed by physicians.
  • Collect and record pertinent clinical and social data for patients through interviews and physical measurements.
  • Register patients, retrieve records, and triage as needed.
  • Collect specimens when necessary and perform EKGs.
  • Provide patient teaching and self-care instructions to promote healthy habits.
  • Order medical supplies to ensure a smooth patient flow.
  • Process referrals, including scheduling, follow-up notification, and report retrieval.
  • Assist in the primary care management of individuals and families through the maintenance of logs and tracking systems.
  • Stay up-to-date on vaccines for children and maintain current licensure.
  • Accurately document and code all services provided within the allotted time frame.
  • Attend in-service and staff development programs to enhance skills and knowledge.

Requirements:

  • Graduation from an accredited program in practical nursing.
  • Current registration to work as a Licensed Practical Nurse in the State of Alabama.
  • At least one year of clinical experience in a pediatric outpatient setting is preferred.
  • Maintains current license on file.
  • Basic computer skills are required.
