The Occupational Therapist at Lakeside Special Care Center will direct all aspects of quality patient care, including evaluation, treatment, treatment planning, goal setting, family education, and documentation according to organization's guidelines, professional standards, and community needs. This role will also serve as a patient and rehabilitation advocate for current and prospective patients, to all internal and external customers while participating in quality assurance and customer service activities.
Key Responsibilities:
Evaluate patients to identify their physical, emotional, and cognitive needs
Develop and implement individualized treatment plans to help patients achieve their goals
Provide education to patients and their families on how to perform daily activities and manage their conditions
Document patient progress and outcomes according to organization's guidelines
Requirements:
Graduate of an approved curriculum in Occupational Therapy
Ability to practice unencumbered and in good standing with all regulatory agencies and licensing boards
Working knowledge of Medicare and other payer sources
Full knowledge of residents' rights
Exude professionalism in presentation and able to work on multiple tasks at the same time
