Job Summary

Aveanna Healthcare LLC is seeking a highly skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team. As a Skilled Nursing Care Specialist, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Deliver and coordinate quality patient care in compliance with physician orders.
  • Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as necessary.
  • Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
  • Participate in and update the nursing care plan to ensure optimal patient outcomes.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
  • Continuing Education as required by state.
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical Requirements
  • Ability to speak, write, read, and understand English.
  • Visual and hearing acuity.
  • Ability to climb stairs and lift 50 pounds.
  • Ability to travel.
