About the Role

We are seeking a skilled and dedicated Medical Professional to join our team as an Outpatient Care Specialist. This is a full-time opportunity that requires 40 hours of work per week, with variable shifts and a mix of office and clinical settings.

Job Summary

The successful candidate will provide high-quality patient care in an outpatient setting, working under the direction of a Registered Nurse and medical orders from Practitioners. Key responsibilities include reviewing and preparing medical records, identifying patient needs, and implementing basic nursing procedures according to plan of care. The ideal candidate will be a Licensed Practical Nurse (LPN) with strong communication and interpersonal skills, able to work effectively in a fast-paced environment.

Key Responsibilities
  • Provide direct and indirect nursing care in an outpatient setting, adhering to established policies, procedures, and guidelines.
  • Review and prepare medical records for assigned patients, ensuring accurate and timely documentation.
  • Identify patient needs and implement basic nursing procedures according to plan of care, including administering treatments, immunizations, and medications as prescribed.
  • Collaborate with RN/Practitioner to revise care plan as needed, demonstrating knowledge, skills, and abilities necessary to provide culturally sensitive care and service.
  • Perform other duties and accept responsibility as assigned within scope.
Required Skills and Qualifications
  • Demonstrated knowledge of and skill in conflict resolution, customer service, interpersonal relations, oral communication, problem solving, teamwork, and written communication.
  • Basic Life Support certification from American Heart Association.
  • Licensed Practical Nurse (LPN) licensure in Hawaii at time of hire.
  • Previous nursing experience preferred, but not required.
