Job Summary

We are seeking a highly skilled and dedicated Speech-Language Pathologist to join our team at Creative Solutions in Healthcare - West. As a key member of our healthcare team, you will be responsible for providing high-quality speech and language therapy services to our residents.

Key Responsibilities
  • Evaluate residents' conditions for swallowing or cognitive deficits, and develop individualized treatment plans to improve their independence and functioning.
  • Provide skilled, medically necessary speech and language therapy assessments, and implement effective treatment plans to address residents' needs.
  • Collaborate with other healthcare professionals to coordinate diagnostic swallowing procedures and provide training and education to residents, family members, and caregivers.
  • Communicate effectively with residents, family members, and healthcare teams to ensure seamless care and support.
  • Manage patient records and maintain accurate and timely billing records and clinical documentation.
Requirements
  • Active/Valid Texas license in Speech-Language Pathology
  • In good standing with all regulatory agencies and licensing boards
  • SNF/Long term care experience preferred
  • Current knowledge of treatment practices
  • Knowledge of Net Health Electronic Documentation System preferred
