Position Overview

The role of the Registered Nurse Case Manager is essential in assessing the multifaceted aspects of patient care. This position requires a comprehensive understanding of psychosocial, environmental, and economic factors that may influence patient outcomes. The RN Case Manager will ensure adherence to physician orders while applying hospital-approved medical necessity criteria to evaluate admissions, care levels, and ongoing patient stays.

Key Responsibilities

  • Conduct thorough assessments to identify patient needs and potential barriers to care.
  • Document reviews and maintain accurate records in compliance with hospital standards.
  • Engage in discussions with healthcare providers regarding alternative care options when necessary.
  • Act as a liaison among patients, caregivers, and the multidisciplinary team to facilitate effective communication.
  • Coordinate with post-acute care providers and third-party payers to ensure seamless transitions and appropriate reimbursement.

Qualifications

Education: Required - Registered Nurse Diploma or equivalent.

Experience: Required - Minimum of 1 year of hospital-based patient care experience. Preferred - Experience in Case Management or Utilization Review, along with knowledge of healthcare delivery systems.

Licensure: Required - Current RN License in the state of practice.

Skills and Competencies:

  • Proficient in computer applications and data entry.
  • Strong verbal and written communication skills, with the ability to convey information clearly.
  • Excellent conflict resolution abilities and adaptability in high-pressure situations.
  • Ability to work independently and collaboratively within a team environment.
  • Demonstrated professionalism, ethical principles, and a strong work ethic.

Physical and Environmental Considerations

This role may require the ability to exert physical force to move objects and involves potential exposure to hazardous materials within a healthcare setting.
