Job Title: Clinical Registered Dietitian II

Adelante Healthcare is seeking a highly skilled Clinical Registered Dietitian II to join our team. As a key member of our integrated care team, you will be responsible for assessing the nutritional status of patients across the lifespan and developing evidence-based nutrition care plans.

Key Responsibilities:
  • Conduct comprehensive nutrition assessments and develop personalized nutrition care plans for patients with various conditions, including Type 2 diabetes, hyperlipidemia, hypertension, metabolic syndrome, and weight management.
  • Collaborate with the integrated care team, including behavioral health consultants, registered nurses, care coordinators, medical assistants, and primary care providers, to ensure seamless patient care.
  • Establish SMART goals with patients and schedule follow-up appointments as needed.
  • Train and onboard new RDs on integration and conduct efficient nutrition consults.
  • Attend and participate in monthly provider meetings to provide nutrition updates and share best practices.
  • Precept dietetic interns and provide guidance and support to newer RDs.
Requirements:
  • Master's degree in Dietetics or a nutrition-related field (preferred) or a Bachelor's degree in Dietetics or a nutrition-related field (required).
  • Three (3) plus years' experience in community or clinical nutrition education programs or completion of a dietetic internship with RD/RDN credentials.
  • Current credentials as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) with the Commission on Dietetic Registration.
  • Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross.
  • Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for all specialty behavioral health locations.
  • Documented excellence in customer service and ability to interact with and instruct employees and clients from beginner to advanced levels.
  • Demonstrated competency in nutrition education methods and techniques.
  • Working knowledge of current Microsoft Office applications and relevant program-related applications.
  • Competency in working with people of various cultures.
  • Experience with motivational interviewing.
  • Evidence of valid Arizona driver's license and current auto insurance.
  • Ability to maintain confidentiality.
  • Prioritization and multitasking skills are required.
  • Bilingual (English/Spanish) strongly preferred.
