About the Role
Hartford HealthCare is seeking a dedicated Nurse Practitioner or Physician Assistant to join our REACH Program, serving geriatric patients in the greater Hartford area.
This full-time position offers an exciting opportunity to work in a home-based setting, collaborating with a multidisciplinary care team to deliver high-quality patient care.
Responsibilities
- Perform routine physical examinations and formulate medical plans to treat patients.
- Develop treatment plans in consultation with physicians and care team members.
- Provide appropriate treatment within the scope of practice and refer complex cases to specialists when necessary.
Requirements
- Unrestricted State of Connecticut Medical License (or eligible).
- APRN (Family, Adult/Geri Nurse Practitioner) or PA training.
- At least two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA).
Benefits and Salary
We offer a competitive salary range of $110,000 - $140,000 per year, based on experience. Our benefits package includes market-leading compensation and benefits, generous paid time off, and opportunities for professional growth and development.
