Complex Care Social Worker - Renal Dialysis Opportunity

We are seeking a skilled Complex Care Social Worker to join our team at Grace Medical Center. As a key member of our interdisciplinary team, you will play a vital role in facilitating discharge planning for patients with complex needs.

Job Summary:

The Complex Care Social Worker will use a multi-step escalation protocol to identify patients with barriers to effective discharge. In collaboration with the interdisciplinary team, you will assume responsibility for escalated patients and work to identify early intervention opportunities to effect a timely and cost-saving discharge.

Key Responsibilities:

  • Systematically gather pertinent information to determine patient needs and develop effective discharge plans.
  • Oversee the efficient management of care and utilization of resources.
  • Collaborate with the multi-disciplinary team to develop and implement plans of care.
  • Communicate effectively with patients, care team members, and community partners.
  • Provide leadership and creativity for programs and projects to enhance care management.

Requirements:

  • Master's degree in social work or related field.
  • Current/Valid license in the State of Maryland; LCSW-C/LCPC required.
  • Minimum of 3 years of experience; 5+ years of experience preferred.
