Child Welfare Case Manager Position

We are seeking a highly motivated Child Welfare Case Manager to join our team. As a key member of our foster care program, you will be responsible for coordinating case management needs of youth and families.

Key Responsibilities:

  • Facilitate Family Assessments and case planning to ensure effective service delivery.
  • Collaborate with service providers to ensure family progress toward treatment goals.
  • Support the development of Individualized Treatment Plans and identify goals for families and children.
  • Provide Supportive Counseling to children, birth parents, and foster parents.
  • Maintain accurate and complete case records, adhering to Social Services Laws and Regulations.

Requirements:

  • Bachelor's degree in social work or a related Human Service discipline.
  • Two years' experience working with youth at risk and families.
  • Valid NYS driver's license and GPSII/MAPP Certification.

Compensation:$65,000 - $85,000 per year, depending on experience and location.
