Physical Therapist

We are seeking a highly skilled Physical Therapist to join our team at Bluestem Village. As a Physical Therapist, you will play a vital role in providing high-quality care to our residents, ensuring their independence and functionality are maintained.
Key Responsibilities- Conduct thorough physical therapy assessments and develop individualized treatment plans to meet the unique needs of each resident.
- Supervise and mentor Physical Therapy Assistants, provisional licensees, and students to ensure they are providing safe and effective care.
- Collaborate with other healthcare professionals to identify and address comprehensive care needs of residents.
- Communicate effectively with residents, families, and healthcare teams to ensure seamless care coordination.
- Participate in discharge planning and community mobility initiatives to ensure residents receive the support they need to thrive.
- Current, unencumbered, active Colorado Physical Therapy License.
- Graduate of an accredited Physical Therapy Program by APTA & CAPTE.
- Experience in providing care in post-acute or long-term care environments (preferred).
- Ability to work independently and as part of a team to achieve exceptional patient outcomes.
As a Physical Therapist at Bluestem Village, you will work in a fast-paced, dynamic environment that requires flexibility and adaptability. You will be responsible for providing care to residents with varying needs and abilities, and will work closely with other healthcare professionals to ensure seamless care coordination.
Physical DemandsThe Physical Therapist role requires frequent squatting, twisting, and reaching, as well as occasional villager handling, transferring, and lifting. You must be able to work in a physically demanding environment and maintain a valid, unrestricted Colorado driver's license.
