Overview

Amedisys is a leading provider of home health and hospice care, and we are seeking a skilled Registered Nurse to join our team. As a Home Health Nurse, you will play a critical role in delivering high-quality patient care in the comfort of their own homes.

Responsibilities

  • Coordinates and oversees all aspects of patients' home care, ensuring timely and accurate documentation.
  • Collaborates with patient care teams to develop and implement care plans that meet the unique needs of each patient.
  • Provides education and support to patients and their families, empowering them to take an active role in their care.
  • Performs head-to-toe physical assessments, checks vital signs, and manages medications to ensure optimal patient outcomes.
  • Administers injections, IV therapy, and tube feedings as needed, and maintains catheters to ensure patient comfort and safety.
  • Supervises LPNs and HHAs, providing guidance and support to ensure high-quality patient care.
  • Regularly communicates patient progress to the clinical manager, physician, and care team to ensure seamless coordination of care.

Qualifications

  • One year of clinical experience as a Registered Nurse (RN), with a minimum of six months of home health experience.
  • Current RN license, specific to the state(s) you are assigned to work.
  • Current CPR certification and valid driver's license, with reliable transportation and liability insurance.
  • Able to assess patient status and identify requirements relative to age-specific needs.
  • Familiarity with general use and functions of the computer, including user names and password concepts, internet, email, and navigation of computer desktop.
