Job Summary:

We are seeking a compassionate Patient Care Advocate to join our team at the Hospice of Muskegon County. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families navigating end-of-life care.

About the Role:

In this dynamic position, you will work closely with patients, families, and healthcare professionals to assess psychosocial needs and develop personalized plans for care. Your expertise in social work will enable you to provide strength-based, person-centered interventions, including supportive counseling, psychoeducation, and anticipatory guidance. You will also collaborate with the interdisciplinary team to provide clinical guidance and consultation, ensuring comprehensive and coordinated care.

Key Responsibilities:
  • Conduct thorough psychosocial assessments to identify potential complicated grief and develop targeted interventions.
  • Develop and implement individualized plans of care, incorporating strengths-based and person-centered approaches.
  • Provide empathetic and non-judgmental support to patients and their families, addressing emotional, psychological, and environmental challenges.
  • Collaborate with the interdisciplinary team to provide clinical guidance, consultation, and education on psychosocial aspects of care.
  • Evaluate the effectiveness of interventions and make adjustments as needed to ensure optimal outcomes.

Requirements:

To succeed in this rewarding role, you must possess a strong foundation in social work principles and practices. A bachelor's degree in social work or a related field is required. Additionally, you should have experience working in a healthcare setting, preferably in hospice or palliative care. Your excellent communication and interpersonal skills will enable you to build strong relationships with patients, families, and colleagues. A valid driver's license is also necessary for travel to patient sites.

Salary Information:

The estimated salary range for this position is $65,000 - $85,000 per year, depending on experience and qualifications.
