Key Responsibilities of a Hospice LPN:
You will report directly to the Administrator or Executive Director.
Deliver direct patient care services to hospice patients and provide supportive care to their families while managing a reasonable caseload.
Assess patient and family needs and assist in the development and implementation of individualized care plans for each patient.Communicate any findings or changes in the patient's condition to the Registered Nurse, interdisciplinary team, and referring physician, documenting all observations accurately.
Participate in staff meetings.Candidate Profile:
Essential Qualifications for Hospice LPN:
Compassionate Heart:
Empathetic, kind, and understanding towards patients and their families.
Comfortable providing care in various settings as needed.
Exhibit flexibility and composure while assisting others during challenging times.
Must possess a valid Licensed Practical Nurse (LPN) license for the state of employment.
Minimum of 1 year of patient care experience in hospice, home health, ICU, geriatric, or oncology preferred.
Additional preferred experience that may enhance your success in this role includes: hospital Staff LPN, Staff LVN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, nursing home, or travel nursing.
Current driver's license and automobile insurance are required.Current CPR certification is mandatory.
