We are seeking a compassionate and dedicated Admissions Registered Nurse (RN) to join our team at BAYADA Home Health Care, working in partnership with At Home Care, Inc.
Responsibilities:- Conduct thorough Start of Care (SOC) Admissions, utilizing OASIS assessments to ensure accurate patient data.
- Develop and implement individualized care plans, adhering to patient needs and preferences.
- Make home visits to clients in designated territories, providing high-quality patient care and education.
- Perform a range of duties, including medication administration, wound care, treatments, and procedures, as prescribed by physicians.
- Monitor patients' conditions, reporting any changes to the Clinical or Client Services Manager.
- Accurately document observations, interventions, and evaluations, utilizing a state-of-the-art touch pad tablet.
- A current license as a Registered Nurse in New York State.
- A minimum of two years of recent, verifiable clinical (medical/surgical) experience.
- Prior home care and OASIS experience is required.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills, prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.

Base Pay, depending upon qualifications and subject to negotiation: $63.00 per point.
