Role Overview

The Clinical Informatics Specialist at Oak Street Health plays a vital role in supporting our primary care providers with clinical documentation, enabling them to focus on delivering exceptional patient care. This position involves assisting providers throughout the patient care journey, from planning daily visits to documenting patient encounters in real-time.

Key Responsibilities
  1. Documenting Patient Encounters
    • Join the provider in the exam room to observe patient visits
    • Document patient encounters in a structured note, including the history of the present illness, assessment, plan, and physical exam
    • Assign appropriate CPT and ICD-10 codes
    • Prepare After Visit Summaries
    • Consult with the provider to ensure accurate and specific documentation
  2. Clinical Documentation Improvement
    • Request and review medical records
    • Leverage Oak Street's population health tools to support clinical documentation improvement
    • Prepare for and support Daily Huddles and Clinical Documentation Reviews
    • Consult with the provider on clinical documentation opportunities
  3. Administrative Support
    • Place orders and referrals
    • Address tasks
    • Support the care team with additional responsibilities related to clinical documentation
Requirements

We are seeking a highly skilled and detail-oriented individual with:

  • Knowledge of medical terminology and common medications
  • Prior clinical experience, including shadowing and/or volunteering
  • Prior scribe or transcription experience (preferred but not required)

The ideal candidate will possess advanced listening and communication skills, strong computer literacy, and the ability to adapt to new workflows and quickly learn new concepts and skills. Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve is required where indicated.
