Job Summary:
Sage Health is seeking a skilled Geriatric Medicine Physician to join our team in Montgomery, AL. As a key member of our care team, you will play a vital role in delivering high-quality patient care and contributing to our mission of providing exceptional healthcare services.
Key Responsibilities:
- Evaluate and treat patients in accordance with standards of care, ensuring a high level of patient satisfaction and outcomes.
- Coordinate patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care, to ensure seamless care transitions.
- Participate in daily huddles with the care team to share medical expertise and collaborate on patient care.
- Work closely with the Lead Physician and Center Administrator to establish a positive and cooperative practice environment.
- Complete accurate and comprehensive medical record documentation, working with a quality-based coder to optimize documentation and improve patient care.
- Follow established policies and protocols defined by Clinical Leadership to ensure high-quality patient care.
- Meet with the Lead Physician to discuss quality of care, review outcome data, and address medical records issues.
- Focus on direct patient care, including patient-facing time and administrative tasks related to patient care.
Requirements:
- Graduate of an accredited MD or DO program from a reputable university.
- Current and unrestricted medical license or willingness to obtain a medical license in the state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required.
- Board Certification or Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine preferred.
- Active and unrestricted DEA license.
- Medicare Provider Number.
- Medicaid Provider Number.
- Demonstrate exceptional interpersonal skills and communication with colleagues and patients.
- Fully engaged in the concept of team-based care model.
- Willingness and ability to learn and adapt to practice in a value-based care setting.
- Superior patient/customer service skills.
- Basic computer skills, including email and EMR.
Preferred Qualifications:
- Knowledge of HEDIS/STARS.
- Knowledge of CAHPS and HOS.
- Experience working in medically underserved or culturally diverse communities.
- Experience working with an electronic health record.
- Experience working in Medicare Advantage Global Risk Models.
