We are seeking a skilled and compassionate Care Management Specialist to join our team at Hackensack Meridian Health. As a key member of our care coordination team, you will play a vital role in ensuring that our patients receive the highest level of care and support.

Responsibilities

  • Referral Management: Process assigned Central Intake referrals, procure necessary documentation, and ensure clean billing.
  • Customer Service: Demonstrate exceptional customer service skills, answer incoming calls, and provide information to referral sources.
  • Authorization and Verification: Perform authorization and verification for identified product lines, enter required information into systems accurately, and ensure timely reimbursement.
  • Communication: Network with facility staff, provide information to referral sources, and screen calls for appropriateness.
  • Problem-Solving: Identify opportunities for cross-selling, utilize intake resources, and drill down to root referral source to obtain primary referral information.
  • Documentation: Maintain accurate account files, enter all information correctly, and obtain necessary documentation.
  • Insurance Verification: Verify member eligibility, ensure correct billing information, and contact benefit providers to gather policy benefits/limitations.
  • Collaboration: Communicate with billing agencies, follow up on pending cases, and organize information, services, and resources.

Requirements

  • Education: High School diploma, general equivalency diploma (GED), or GED equivalent programs.
  • Skills: Proficient in Google Space, strong customer service skills, and ability to lift a minimum of 10 lbs.

Preferred Qualifications

  • Experience: One (1) or more years of healthcare experience, home care experience, and billing/insurance authorization experience.
