Alternate Solutions Health Network LLC is seeking a skilled Occupational Therapist to join our team in Oakton, Virginia.

We are looking for a compassionate and detail-oriented Occupational Therapist to provide high-quality care to our patients in the comfort of their own homes. As a member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to develop and implement personalized plans of care.

• Provide skilled occupational therapy services to patients in their homes, as ordered by the attending physician.
• Conduct thorough evaluations to assess patients' level of functioning and develop individualized plans of care.
• Implement evidence-based interventions to improve patients' range of motion, muscle strength, and coordination.
• Collaborate with patients and their families to educate them on disease management, prevention, and healthy lifestyle choices.
• Communicate effectively with healthcare professionals, including physicians, nurses, and social workers, to ensure seamless care coordination.
• Document patient progress and outcomes accurately and in a timely manner, adhering to agency protocols and regulatory requirements.

• Current licensure as an Occupational Therapist in the state of employment.
• Minimum one year of experience as an Occupational Therapist in an acute care setting, with home care experience preferred.
• Valid driver's license and auto insurance in your name as a driver.
• Ability to meet the physical demands of the job, including lifting, bending, and standing for extended periods.
