About the Role:

The MDS Nurse is a key member of our skilled nursing facilities team, responsible for conducting federally mandated assessments of patients to promote their physical and emotional well-being. As a registered nurse, you will collect and compile data into a thorough report to inform care plans and ensure compliance with Medicare and Medicaid requirements.

Key Responsibilities:

  • Conduct initial and periodic interviews with residents and their families to gather information using the Resident Assessment Instrument (RAI)
  • Collect and analyze data on residents' mood, behavior patterns, cognitive ability, and nutrition needs
  • Develop and implement individualized care plans in collaboration with social services, dieticians, rehab specialists, and medical staff
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed
  • Ensure compliance with Medicare and Medicaid requirements and ethical standards

Requirements:

  • MA Licensed Practical Nurse (RN) license
  • 1 year of prior experience working as an RN in a skilled LTC setting preferred
