We are seeking a skilled Home Health Physical Therapist to join our team. As a Traveling Healthcare Professional, you will have the opportunity to work in a new location, providing quality care to patients in need.

The estimated weekly compensation for this role is $2,520 - $2,706 per week, which includes both taxable wages and tax-free reimbursements for meals, housing, and incidentals.

Job Description

This contract assignment requires previous experience with OASIS/Medicare Certified Visits. As a Home Health Physical Therapist, you will have a designated territory of coverage with a set amount of patients.

  • Work as an independent contractor, providing physical therapy services to patients in their homes.
  • Conduct assessments and develop treatment plans tailored to each patient's needs.
  • Collaborate with other healthcare professionals to ensure seamless care delivery.
Required Skills and Qualifications

To be successful in this role, you must have:

  • A graduate degree from an accredited PT program.
  • Licensure as a Physical Therapist in West Virginia.
  • CPR certification or ability to obtain.
  • Previous experience with computer charting.
  • 6 months of prior Home Health Care experience.
  • A valid U.S. driver's license and reliable transportation.
